Liberty Village Assisted Living in Tomah, Wisconsin, provides two levels of care: Assisted Living for residents who need everyday support and Memory Care for those living with various stages of dementia, including advanced conditions. The community can coordinate wound care, oxygen therapy, special diet planning, and monitoring for residents who manage diabetes with oral medications.
Care delivery is backed by an on-call nurse 24 hours a day, an awake night shift, and staff who have completed background checks. Residents can request help through in-room pull cords or wearable PERS devices that alert staff immediately. On-site services include hospice coordination, regular nursing visits, podiatry, incontinence support, medication management and support, ambulatory and memory care programming, housekeeping, personal care, wound treatments, and optional money-management assistance. Staff can provide full assistance with bathing, dressing, and toileting, standby supervision during meals, and transfer support ranging from walker guidance to two-person lifts.
Apartments are designed for ease of use and safety. Some residences offer a private bathroom, a kitchenette for simple meal prep, individual heating and cooling controls, and basic cable service, and all receive routine housekeeping. Not every unit includes every feature.
Meals follow a set daily menu with the option to order off menu when desired, and snacks are available throughout the day. Dietary modifications are accommodated as part of the community’s focus on special diets and physician-ordered nutrition plans.
Amenities include resident parking, wheelchair-accessible common areas, an on-site barber and beauty salon, an exercise room, a movie room, internet access, communal gardens, and a library. Pets are allowed; it’s a good idea to check with the community during your visit to see if your pet will be allowed.
