Heritage Court – Menomonee Falls is a dedicated memory care community that serves residents living with Alzheimer’s disease and other forms of dementia. Care is provided around the clock by staff members who have received dementia-specific training, and licensed nurses oversee medication administration, routine vital-sign checks, diabetic care, and coordination with outside physicians and therapists. The community maintains individualized care plans that are reviewed regularly so that any changes in behavior, cognition, or medical status are addressed quickly. Emergency response systems are in place throughout the building, and staff members are available 24/7 for urgent needs or unscheduled assistance.
In addition to medical oversight, the community offers everyday support with bathing, dressing, continence management, and mobility. Caregivers cue residents through daily schedules, guide them to structured small-group activities designed to stimulate memory, and accompany them to on-site therapy sessions when ordered. Scheduled transportation is available for off-site medical appointments, and the staff coordinates pharmacy deliveries, lab work, and visiting practitioner services to minimize resident disruption. A secured environment with monitored exits reduces wandering risk while still providing access to enclosed outdoor walking paths.
Accommodations include a selection of private and companion suites equipped with bathrooms, emergency call systems, and basic cable. Each residence is designed for ease of navigation, and some suites offer additional features such as individual climate controls or courtyard views; however, not all accommodations have all the features mentioned. Furnishings can be provided by the community, or residents may bring personal items to create a familiar setting. Weekly housekeeping and linen services are included, and maintenance staff handle routine repairs.
Meals are prepared on-site in a commercial kitchen, and menus are developed with input from dietitians to meet the nutritional needs of older adults with cognitive impairment. Three daily meals plus snacks are served in a comfortable dining room where staff members provide cueing and assistance as needed, including modified utensils and texture-altered diets for residents with swallowing difficulties. Hydration stations remain available throughout the day, and families are encouraged to share favorite recipes for occasional menu inclusion.
