HEARTIS | BROOKFIELD offers two levels of residential care. Assisted Living is designed for older adults who want an independent lifestyle but may need scheduled or unscheduled help with bathing, dressing, transfers, medication reminders, or other activities of daily living. Memory Care is structured for people living with Alzheimer’s disease or other forms of dementia; it provides a secured environment and programming intended to reduce confusion, support familiarity, and maintain daily routines while still allowing as much personal choice as possible.
Additional medical support rounds out the care model. The community accepts residents for respite or short-term stays, accommodates diabetes management plans, and provides access to a speech therapist when needed. On-site services include visits from a dentist, podiatrist, optometrist, physical, occupational, and massage therapists, as well as nurses who oversee medication management and general health monitoring. Staff members are trained in ambulatory and incontinence care, hospice care is available for end-of-life comfort, and the building is secured specifically to support residents who experience memory loss.
Apartments range from studio to larger one-bedroom layouts. Depending on the selected unit, residents may find kitchenettes, wheelchair-accessible showers, cable or satellite TV connections, Wi-Fi access, and individual climate control with air conditioning. Some residences overlook landscaped outdoor areas, and parking is available for those who keep a car. Pets are welcome in designated units, though not every accommodation includes pet-friendly features or every amenity listed.
Dining follows a flexible, all-day service model. A professional chef prepares three daily meals, with vegetarian and vegan dishes always on the menu. Restaurant-style table service allows residents to choose entrees at each seating, and guest meals can be arranged for visiting family or friends. Snacks are available throughout the day for those who prefer smaller, more frequent servings.
Residents can take part in a full calendar of on-site and off-site activities. The campus includes indoor and outdoor walking paths, a swimming pool, jacuzzi, raised garden beds, an activity center, and a library with computer lounge. Structured programs such as fitness classes, yoga and chair yoga, reminiscence sessions, intergenerational visits, live music, karaoke, birthday parties, dances, happy hours, and organized social outings help residents stay active and socially connected. Clubs and informal communities meet regularly to share interests.
