Oak Park Place – Baraboo provides Assisted Living, Independent Living, and Memory Care programs on one campus, allowing residents to transition between levels as their needs change. The community supports residents who require wound care, oxygen therapy, special diets, or diabetes management—whether controlled by oral medication or insulin injections. It also accepts individuals with advanced dementia conditions and offers a specially designed memory-care environment intended to lessen confusion and wandering.
Round-the-clock clinical oversight is standard. An RN or LPN is on site, with a nurse on call 24 hours a day. Care plans can include full assistance with bathing, dressing, and toileting, along with some assistance at mealtimes. Transfer support ranges from standby supervision to a two-person or full Hoyer lift when necessary, and staff remain awake overnight. Pull cords in apartments and personal emergency response systems link directly to caregivers. Background-checked personnel, an activity director, and on-site occupational and physical therapy round out the support structure.
Apartments are offered in several layouts. Depending on the unit selected, features may include a private bathroom, kitchenette, individual climate controls, cable service, and regular housekeeping. Prospective residents should confirm which features are included in each available floor plan.
Dining options cover a fixed daily menu as well as made-to-order selections. Snacks are available throughout the day, and meals can be served in the main dining room, a private dining space for family gatherings, or delivered to the apartment when needed. The culinary team handles physician-ordered diet modifications and management for conditions such as diabetes or texture-modified requirements.
Residents have access to on-site parking, wheelchair-accessible common areas, a barber and beauty shop, exercise room, wellness center, Internet connectivity, regularly scheduled religious services, and a library for reading or quiet study.
