The Haven in Stone Oak is dedicated exclusively to memory-care, serving residents with Alzheimer’s disease and other forms of dementia. Licensed nurses oversee round-the-clock clinical supervision, handle medication administration, and coordinate directly with residents’ physicians to keep care plans current. The care team is trained in dementia-specific techniques such as redirection and cueing, and they monitor vital signs, weight, and behavioral changes daily. On-site medical oversight includes routine health assessments, fall-risk evaluations, and wound care when prescribed, while outside providers such as podiatrists, psychiatrists, and physical therapists visit the community as needed to deliver additional services.
Beyond medical treatments, the community supplies structured support for activities of daily living—bathing, dressing, grooming, and continence management—tailored to each resident’s cognitive abilities. Caregivers are on call 24/7, and every suite is equipped with an emergency response system linked to the nursing station. The Haven also offers secure indoor and outdoor areas to minimize wandering risk, daily cognitive stimulation programs, and family support groups. Transportation for medical appointments is arranged when off-site visits are required, and electronic documentation keeps families and physicians informed of changes in real time.
Accommodations range from private studios to companion suites, each designed with simplified layouts and clear sight lines to reduce confusion. Rooms can include features such as individual climate controls, cable access, accessible bathrooms with grab bars, and built-in wardrobes, although not all accommodations have all the features. Housekeeping, laundry, and maintenance are handled by staff, and residents are encouraged to personalize their living spaces with familiar belongings to promote comfort and recognition.
Dining is provided three times a day in a communal setting, with menus developed by a registered dietitian to address the nutritional needs common in dementia care, including modified textures and finger-food options. Staff members offer verbal prompts and hand-over-hand assistance when necessary, and they monitor fluid intake to guard against dehydration. Snacks are available throughout the day, and special diets—such as low-sodium or diabetic—are accommodated on request.
