Autumn Leaves of Edmond is a memory-care–only community designed to support residents living with all stages of dementia, including advanced conditions. Core services center on cognitive support, medication management and daily clinical oversight. The community accepts both long-term residents and those seeking short-term respite stays. A registered nurse, licensed practical nurse and on-call nursing coverage are in place, while visiting physicians, diabetes-care oversight and speech therapy broaden the medical reach.
Additional layers of care include 24-hour staffing, a secured perimeter with fenced front and rear areas, and personal emergency response systems that alert staff. Residents needing full assistance with bathing, dressing, eating or toileting receive hands-on help, and the team can manage incontinence and hospice needs. Mobility support accommodates walkers, wheelchairs and standby transfers, as well as one- or two-person physical assists. On-site occupational and physical therapy, podiatry services and structured memory-care programming round out the clinical offerings.
Accommodations range from private to companion suites, all of which are handicap accessible; many feature wheelchair-friendly showers, individual climate control and in-room Wi-Fi, though not every unit includes every feature. The building offers common spaces such as a central fireplace lounge, a piano area for planned music sessions and outdoor sitting areas. Limited parking is available for residents and guests, and the community maintains a pet-friendly policy for select apartments.
Dining is provided through a set menu at scheduled mealtimes, with the option to order alternate selections if desired. Snacks are available throughout the day, and the culinary team collaborates with clinical staff to deliver modified or therapeutic diets when prescribed.
Residents have on-site access to a barber and beauty shop, a wellness center for light exercise and health programming, and communal gardens that serve as a secure outdoor space for supervised walks and activities.
