The Hearth at Greenpoint provides both Assisted Living and Independent Living, allowing residents to choose the level of day-to-day support that matches their needs. Assisted Living residents have licensed nurses—RNs and LPNs—available around the clock for medication administration, scheduled health checks and diabetes monitoring. The community can also accept individuals with advanced dementia, coordinating closely with families and physicians to tailor care plans. Independent Living residents retain more autonomy but can add medication monitoring if daily reminders or oversight become necessary.
Additional clinical services are delivered on site. Residents who need help with continence care, ambulatory assistance or memory-related interventions have access to trained staff and visiting clinicians. Occupational and physical therapists develop therapy schedules without requiring off-site trips, while a consulting podiatrist addresses routine and diabetic foot concerns. Staff provide full assistance for bathing, dressing, eating and toileting when ordered, and they are trained in safe walker use and one-person transfers to maintain mobility while minimizing fall risk. Medication management, from routine prescriptions to as-needed doses, is handled and logged by licensed nurses.
Apartment styles vary; some have private bathrooms, kitchenettes and basic cable, while others offer a more streamlined layout, so it is best to confirm specific features during a tour. Housekeeping is provided for all units, and laundry service is available weekly. Independent Living residents can opt into medication-management monitoring, housekeeping and laundry on a scheduled basis, allowing them to remain self-reliant while still having a safety net.
Meals follow a set menu prepared by dietary staff who accommodate physician-ordered restrictions and resident preferences, including low-sugar or carbohydrate-controlled options for diabetes. Snacks are available throughout the day, and a private dining room can be reserved for family visits or special events. The culinary team works with clinical staff to adjust textures and portion sizes for residents who require diet modifications.
Resident parking is offered for those who still drive, and scheduled transportation helps others reach local appointments or shopping centers. Pets are allowed; it’s a good idea to check with the community during your visit to see if your pet will be allowed.
