The Chelsea at Sparta delivers two primary levels of care—Assisted Living and Memory Care—with both programs designed to address a wide range of clinical needs. Staff members are trained to support residents who wander during the day or night, those at risk for elopement, and individuals who are awake throughout the night. Oxygen use, special diets, and diabetes management through oral medications, injections, or sliding-scale insulin are all accommodated. The Memory Care neighborhood is structured to serve residents with early, mid, or advanced dementia, providing consistent routines and supervision tailored to cognitive decline.
Clinical oversight is supplied by on-site RNs or LPNs, a 24-hour nurse on call, and an awake night shift. All employees undergo background checks, and each apartment is connected to a personal emergency response system that alerts staff. Medical professionals visit for podiatry, and a therapy room supports rehabilitation goals. Full assistance is available for bathing, dressing, eating, and toileting, and the community can manage transfers ranging from standby help with a walker to two-person assist for wheelchair users. An activity director coordinates structured programs that align with residents’ care plans.
Apartments include practical features such as private bathrooms, individually controlled heating and cooling, cable access, housekeeping, and scheduled laundry services, though not every unit offers every feature. Room layouts allow for personal furnishings so residents can tailor the space to their comfort levels while still receiving needed support.
Meals follow a set menu prepared to meet nutritional guidelines, with snacks available throughout the day. The kitchen can provide mechanical-chopped or puréed textures and other diet modifications under physician order. A private dining room is available for family visits and special occasions, making it easier to observe dietary plans without disruption to social routines.
Residents have access to on-site parking, wheelchair-accessible common areas, a barber and beauty shop, movie room, wellness center, internet, chapel services, concierge assistance, a library, and various special-interest clubs. Pets are allowed; it’s a good idea to check with the community during your visit to see if your pet will be allowed.
