Azria Health Centennial Park offers three distinct levels of residential care. Seniors who prefer an Independent Living arrangement can maintain their usual routines while having access to staff and services on site. Assisted Living residents receive daily support with personal care tasks, while the Memory Care program is designed for individuals living with Alzheimer’s disease or other forms of dementia. Because these care types are grouped on one campus, residents can move between service levels if their needs change, reducing the likelihood of having to relocate elsewhere.
The community backs its care programs with medical oversight and safety measures. Short-term respite stays are available, and every resident can receive regularly scheduled wellness checks as well as diabetes monitoring and management. Keyed entries and on-site security personnel help control building access, and each residence is equipped with an emergency call system that links directly to staff. Medical services include round-the-clock licensed nurses, hospice coordination, incontinence and ambulatory care, and counseling for residents and families. With nurses on the premises 24 hours a day, the team can respond quickly to changes in condition and coordinate with outside providers when necessary.
Apartments and suites vary, but many offer features such as in-unit washers and dryers, cable TV connections, and parking for personal vehicles. Outdoor areas include resident-tended gardens and spaces for intergenerational programs, giving families opportunities to visit and participate in activities. All primary utilities are covered in the base rate; telephone, cable, and internet services are available for an additional fee. Because floor plans and feature sets differ, not every accommodation contains every listed amenity, so it is advisable to review specific units during a tour.
Dining services provide three daily meals with menu options that can be modified for special diets, including low-sugar or low-sodium selections when required. Meals are served in a communal dining room, and staff monitor nutritional intake for residents who need assistance. Snacks and beverages are available outside scheduled mealtimes, and special holiday or themed meals are offered throughout the year.
Residents can take part in an activity calendar that includes an on-site center for arts and crafts, regularly scheduled speaker series, a library and computer lounge, happy hours, game and movie nights, health and wellness programs, and organized social outings.
