Chestnut Terrace II is a dedicated memory care community that concentrates on the needs of residents living with Alzheimer’s disease and related dementias. Licensed nurses and care associates are on duty around the clock to handle medication management, monitor health conditions, and respond to changes in status. Care plans are individualized and reviewed regularly with residents’ physicians and families, and the community uses secure entryways, wander-alert systems, and ongoing cognitive assessments to minimize safety risks.
Beyond core medical oversight, the community offers daily assistance with bathing, dressing, toileting, mobility, and other routine tasks. Structured cognitive programs, small-group activities, and one-on-one engagement are scheduled to help preserve abilities and reduce anxiety. Transportation to outside medical appointments, access to visiting rehabilitation therapists, and coordination with hospice or home health providers are available when ordered by a physician. Housekeeping, laundry, and maintenance services are included so residents can focus on health and social interaction.
Accommodations consist of private and companion suites designed with dementia-friendly layouts, clearly marked bathrooms, and emergency call systems. Rooms may feature individual climate controls, built-in wardrobes, and cable or internet connections, although not every unit includes every feature. Regular safety checks and weekly housekeeping keep living areas orderly, and maintenance staff handle repairs promptly.
Meals are prepared on site three times a day, with menus developed under dietitian guidance to support balanced nutrition and accommodate physician-ordered restrictions such as low-sodium, diabetic, or pureed diets. Residents eat together in a central dining room to encourage socialization, and staff provide cueing or full assistance when needed. Snacks and hydration stations are available throughout the day, and families can discuss specialized dietary needs with the culinary manager.
