Chestnut Terrace offers a dedicated memory care program designed for residents living with Alzheimer’s disease or other forms of dementia. Licensed nurses oversee care around the clock, manage medications, monitor vital signs, and coordinate with physicians to manage chronic conditions. The community can arrange on-site visits from primary care providers, podiatrists, rehabilitation therapists, and hospice agencies, helping residents receive most of their medical support without leaving the building. An electronic health record system tracks changes in condition so the care team can respond quickly to medical concerns.
Beyond core medical services, the staff assists with daily activities such as bathing, dressing, grooming, mobility, and continence. Individualized care plans are reviewed regularly and adjusted as conditions change. The community maintains secure indoor and outdoor areas to reduce exit-seeking behavior and supports cognitive health through structured programming that targets memory, orientation, and social engagement. Routine wellness checks, fall-prevention strategies, and scheduled transportation to outside medical appointments add an extra layer of safety and continuity of care.
Residents can choose from private or companion suites, each with its own bathroom, climate control, and emergency call system, though not all accommodations include every feature mentioned. Housekeeping, laundry, and maintenance services are provided, and basic utilities, cable, and Wi-Fi are included in the monthly fee. Some suites are large enough to accommodate personal furniture, and select units overlook landscaped courtyards. Pets are welcome in certain areas of the community, and it’s a good idea to check with the community during your visit to see if your pet will be allowed.
Dining services include three daily meals prepared by culinary staff under the guidance of a registered dietitian. Menus rotate regularly and can be modified for diabetic, low-sodium, or texture-modified requirements, with special attention to dementia-friendly plate presentation and calm dining environments that reduce confusion. Staff provide cueing or direct assistance during meals for residents who need help maintaining adequate nutrition. Snacks and beverages are available throughout the day, and families can reserve a private dining space to share a meal or celebrate special occasions with their loved one.
