StoryPoint Rockford provides three levels of residency—Assisted Living, Independent Living, and Memory Care—so residents can remain in a familiar setting as needs change. The clinical team supports a wide range of medical conditions, including residents who wander day or night, elopement risks, combative or other behavioral challenges, and those who stay up all night. Care is in place for wound management, oxygen therapy, specialized diets, and diabetes treated by either oral medication or insulin injections. Younger adults under 60 and individuals with advanced dementia are accepted, allowing families to find specialized oversight that many communities do not offer.
An RN or LPN is on-site around the clock, with a second nurse on call 24/7 for backup. Caregivers are awake overnight, and every employee passes a background check. Apartments are equipped with pull cords and optional personal emergency response systems; exterior doors are alarmed and staff maintain 24-hour security coverage. Full-assistance is available for bathing, dressing, eating, and toileting, and the team manages transfers ranging from standby supervision to two-person or full Hoyer-lift support, including bed-bound care. On-site podiatry visits are routine, and residents with psychiatric diagnoses are accepted following an assessment by the nursing team.
Apartments are offered in several floor plans, some featuring private bathrooms, kitchenettes, furnishings, individual climate controls, and cable service; not every unit includes every feature, so it is best to confirm availability during a tour. Residents have access to a set daily menu as well as made-to-order selections, and snacks are stocked throughout the day. Therapeutic and texture-modified diets are prepared under the direction of the clinical and culinary teams to meet physician orders.
Independent Living residents keep their autonomy while benefiting from medication-management monitoring, weekly housekeeping, personal laundry service, and a monthly meal plan that covers regular dining room use. These supports allow residents to maintain a low-maintenance lifestyle while still having rapid access to the licensed nursing staff when health needs emerge.
The community offers resident parking, is fully wheelchair accessible, and welcomes pets. It also features a barber and beauty shop, exercise room, movie room, wellness center, high-speed internet access, communal gardens, and regular on-site religious services, including Christian worship options. Scheduled transportation is provided, and the campus is close to public transit for additional convenience. It’s a good idea to check with the community during your visit to see if your pet will be allowed.
