Wellbrooke of Wabash offers two levels of long-term care: Assisted Living for residents who need help with everyday routines and Memory Care designed for individuals living with Alzheimer’s disease or other dementias. Memory Care is set up to accept residents who wander during the day or night, exhibit exit-seeking or combative behaviors, are under 60 years old, or are coping with advanced dementia diagnoses. Clinical oversight is a priority: registered nurses and licensed practical nurses are on the staff roster, a nurse is on the premises around the clock, and the community manages medication administration for all Assisted Living and Memory Care residents.
Additional medical support is delivered on site by physical, occupational, and speech therapists, a podiatrist, a dentist, and hospice providers. Incontinence management, ambulatory care, and full-assistance bathing, dressing, or toileting are available whenever needed; residents who can eat on their own are encouraged to do so to maintain independence. Staff members can accommodate walker and wheelchair mobility, provide standby supervision, or complete a one-person transfer. Security measures include 24-hour staffing, pull-cord alert systems, building monitoring, and employee background checks. An activity director coordinates daily cognitive and social programs that match each resident’s care plan.
Apartments are designed for privacy and comfort. Depending on the unit selected, a resident may have a private bathroom, a kitchenette, independent heating and cooling controls, basic cable, and the option to paint or decorate the space. Because floor plans vary, not every accommodation includes every feature, so prospective residents are encouraged to verify specific details during a tour.
Three daily meals are served in the dining room, with a rotating set menu complemented by on-order selections for personal preferences. Nutritional support continues throughout the day with scheduled snack times and additional refreshments available on request. For residents in Independent Living, the community offers medication-management monitoring to ensure doses are taken correctly without removing day-to-day autonomy.
Resident parking is available for those who still drive, and all common areas and exterior paths are wheelchair accessible. High-speed Internet access keeps residents connected to family, friends, and telehealth resources. Scheduled transportation is provided for medical appointments, shopping, and local events, and additional rides can be arranged through the front desk.
