Brookdale Chatfield delivers three distinct levels of support—Independent Living, Assisted Living, and Memory Care—under one roof so residents can age in place without relocating. The community is equipped to manage complex medical profiles common to each level of care, including residents who wander at any time of day, present elopement risk, show behavioral issues, remain up all night, or need wound care and oxygen therapy. Diabetes care is available for oral medications, injections, or sliding-scale insulin, and staff can accommodate special diets as well as residents younger than 60. The Memory Care program also accepts individuals with advanced dementia, providing structure and oversight tailored to changing cognitive needs.
Care delivery is backed by licensed nurses on-site, a 24-hour nurse on call, and an awake night staff. All employees undergo background checks, and the building features door alarms, fenced outdoor areas, pull cords, and personal emergency response systems that alert staff around the clock. On-site medical services include podiatry, audiology, therapy space, and acceptance of psychiatric conditions. Residents who need help with activities of daily living can receive full assistance with bathing, dressing, eating, and toileting. Mobility needs are addressed with options ranging from standby support and one- or two-person transfers to full Hoyer-lift assistance for bed-bound individuals.
Apartments vary, but many offer private bathrooms, kitchenettes, furnished layouts, individual climate control, cable connections, and the option to paint or decorate. Housekeeping, in-unit laundry, and scheduled laundry services are available, though not every accommodation includes every feature. Independent Living residents may add medication-management monitoring, housekeeping, laundry, and a monthly meal plan to their service package if desired.
Dining services are flexible. Residents can order meals as needed, request in-room dining, or reserve the private dining room for family events. Snacks are stocked throughout the day, and the culinary team can prepare mechanically chopped or puréed textures as well as other physician-ordered diet modifications. The goal is to maintain nutrition, support medical needs, and allow residents to choose when and how they eat.
