Carlton Senior Living Memory Care of Contra Costa is dedicated exclusively to Memory Care, concentrating on residents who live with Alzheimer’s disease, other forms of dementia, or cognitive decline. The community is prepared for complex medical needs that often accompany memory loss, caring for residents who wander at any time of day, remain up all night, require oxygen, follow special diets, live with diabetes (both oral medication and insulin injections), or are younger than 60. Advanced dementia conditions are welcomed, and individualized care plans help staff manage changing behaviors, medication schedules, and clinical monitoring so residents remain as stable and comfortable as possible.
Around-the-clock support reinforces this medical focus. A registered nurse or licensed practical nurse is on site, with an additional nurse on call 24 hours a day. All team members undergo background checks, and the overnight shift is fully awake for constant supervision. The building’s entrances are alarmed, front and rear grounds are fenced, and Wander Guard technology plus personal emergency response systems instantly alert staff if a resident attempts to leave or needs help. Full-assistance is offered for bathing, dressing, eating, and toileting, while transfer capabilities range from supervision and walker aid to two-person or full Hoyer lift transfers for bed-bound individuals. On-site podiatry simplifies foot care, and the community accepts residents with psychiatric diagnoses, ensuring a seamless blend of physical and behavioral health services under one roof.
Residents choose from studio and companion layouts, and while every apartment is designed for safety, not all units include every feature. Some offer a private bathroom, others share, and several suites add independent temperature controls or cable television connections. Housekeeping, linen changes, and personal laundry are provided for all Memory Care residents, but Independent Living tenants can also opt into medication-management monitoring, housekeeping, and laundry on an à-la-carte basis.
Dining follows a set menu that rotates seasonally, with options for diabetic, low-sodium, mechanical-soft, and other physician-ordered modifications. Three meals a day are served in a central dining room with snacks available throughout the day to curb hunger or accommodate unpredictable appetites common in dementia care. Staff remain present to cue, assist, or provide full feeding support so each resident maintains adequate nutrition and hydration.
The campus is wheelchair accessible and smoke-free indoors, with designated outdoor areas for smokers. Residents have access to a barber and beauty shop, exercise room, movie room, communal gardens, high-speed internet, and on-site religious services. Scheduled transportation, proximity to public transit, and an activity director who coordinates off-site outings further expand daily options for engagement and social connection.
