Oakmont of Concord provides both Assisted Living and Memory Care, allowing residents to age in place even as needs change. The clinical team is equipped to manage oxygen therapy, tailor special diets, and support all levels of diabetes care—oral medications, injections, and sliding scale. The Memory Care neighborhood accepts residents with advanced dementia conditions and is designed to maintain safety while supporting cognitive health.
Around-the-clock oversight is supported by an RN/LPN on staff and a 24-hour nurse on call. Caregivers remain awake overnight, and every employee undergoes background checks. Pull cords and personal emergency response systems signal staff immediately, while secured, fenced perimeters and door alarms help prevent wandering. On-site services include 12- to 16-hour nursing coverage, medication management and coordination, occupational and physical therapy, podiatry, dentistry, ambulatory and incontinence care, and hospice support. Full-assistance is available for bathing, dressing, eating, and toileting. Residents who use walkers or wheelchairs, require one- or two-person transfers, or need a Hoyer lift can be safely accommodated; bed-bound individuals are also accepted.
Apartments offer options such as private bathrooms, kitchenettes, and cable service, though not every unit contains every feature. Rooms are wheelchair accessible, and an enclosed courtyard gives Memory Care residents a secure outdoor option.
Three daily meals are prepared on site, and menus can be adjusted for physician-ordered diets. Snacks are available throughout the day. Independent Living residents can opt into medication-management monitoring, weekly housekeeping, and personal laundry services.
Residents have on-site parking, and the building is fully wheelchair accessible. A barber/beauty shop, exercise room, and wellness center are on the premises, and high-speed internet access keeps residents connected. Scheduled transportation is offered for medical appointments and errands, and the property is also close to public transit. Smoking is permitted outdoors in designated areas.
