The Brookfield at Oaktree delivers two primary levels of care: assisted living and a dedicated memory care program. Licensed nurses are on-site daily, and caregivers are available around the clock to help with bathing, dressing, mobility and medication management. Each resident receives an individualized service plan that is reviewed regularly with input from the resident, family and physician. For those living with Alzheimer’s disease or other forms of dementia, the secured memory care neighborhood offers structured routines, heightened supervision, behavior monitoring and staff trained in validation techniques. Emergency call systems are in place throughout the community, and staff are trained to respond immediately to urgent medical situations.
Additional health services include scheduled wellness checks, coordination with outside physicians, and transportation to local medical appointments. Physical, occupational and speech therapy can be arranged on-site, and the community works with local hospice and home-health agencies when higher acuity support is needed. Pharmacy coordination helps ensure timely refills and dose changes, while electronic medication records reduce errors. Respite stays are accepted when space allows, giving short-term caregivers a break and potential residents an opportunity to evaluate the community.
Residents choose from studio, one-bedroom and companion-style apartments equipped with private baths, individual climate controls and basic cable hookups; some floor plans add kitchenettes, walk-in closets or patio access, but not all accommodations have all the features. Safety elements such as grab bars, step-in showers and smoke detectors are standard, and an emergency pull-cord system links directly to the care team. Common areas include lounges, activity rooms, a beauty salon, landscaped courtyards and a secured garden for memory care residents.
The dining program provides three daily meals with restaurant-style service, plus snacks available throughout the day. Menus are reviewed by a dietitian and can be modified for diabetic, low-sodium, mechanical-soft or pureed diets. Memory care meals incorporate smaller portion sizes, cueing techniques and adaptive utensils to encourage independence. Residents may invite guests to dine for a modest fee, and a private dining room can be reserved for family events when available.
