Sage House – 35th operates as a residential care home and is equipped to manage a broad range of medical needs. The team can look after residents who wander or attempt elopement, those with combative or other behavioral concerns, individuals who stay up all night, and people living with advanced dementia. Skilled caregivers provide wound care, oxygen management, and specialized diets, and they support residents under 60 as well as diabetic residents—whether on oral medications, injections, or sliding-scale insulin. The community also accepts residents with psychiatric conditions, ensuring that complex medical and behavioral profiles can be addressed on site.
Around-the-clock oversight is delivered by an RN or LPN on staff and a nurse on call 24 hours a day. Caregivers remain awake through the night, doors are alarmed, the rear area is fenced, security staff are present 24/7, and a PERS system alerts the team to emergencies. Residents receive full assistance with bathing, dressing, eating, and toileting, and the staff can accommodate transfers ranging from standby support and one- or two-person assists to full Hoyer lift transfers for bed-bound individuals. Comprehensive background checks are conducted on all employees to help maintain safety and consistency of care.
Private and semi-private rooms are available, and some suites come with private bathrooms, independent temperature controls, cable service, and the option to paint or decorate. Furnishings are provided, housekeeping is scheduled routinely, and laundry services are handled on a set timetable. Because configurations differ, not every room includes every feature, so prospective residents should confirm specific details during a tour.
Dining offers both set menus and made-to-order choices, with snacks available throughout the day and the option for in-room meal service. The culinary team supports a variety of diet modifications, including gluten-free, vegetarian, mechanical soft, puréed, and thickened-liquid diets, and coordinates closely with medical staff to adjust meals for diabetic or other therapeutic requirements.
Residents have access to parking, internet, wheelchair-accessible common areas, communal gardens, and on-site religious services. Outdoor smoking areas are designated, and the community is pet-friendly; however, it’s a good idea to check with the community during your visit to see if your pet will be allowed. Scheduled transportation helps residents reach medical appointments, shopping, or other local destinations.
